Fertility Solutions Post Operative Audit Form

Objectives of the Post Operative Audit:

To assess the post operative recovery period on FS patients who have undergone surgery
To identify any deficiencies in patient post operative recovery requiring follow-up action
To demonstrate any improvements in post operative patient recovery in subsequent audits.

Method and Sample:

Audit of patients who have had surgery relating to fertility treatment.

A Questionnaire will be posted to all patients who have had a specific operative procedure with FS along with a stamped self addressed envelope to encourage compliance.

Types of Surgical Procedures to be Audited:

Testicular Surgery
Ovum Collection Procedure

Operative Procedure Review
